INITIAL CONSULTATION (No charge)
Meeting(s) and phone consultations with family or individual with disability, to provide assistance.
COMPREHENSIVE SIX MONTH PLANNING PERIOD ($4100)
Meetings with Life Services staff for Planning Journal and Letter of Intent development.
Completion of the Planning Journal and Letter of Intent requires decisions regarding:
Trusts: Immediate or Future Funding
Family Wills and Estate Documents
Potential development of a Family Created Trust
Annual or Custom Advocacy Service Programs
Keeping Connected Status
This program is geared to parents and relatives planning for and with their relative with disabilities.
IMMEDIATE USE OF LIFE SERVICES TRUSTS TO MAINTAIN OR OBTAIN GOVERNMENTAL BENEFITS (Initial meeting free, fee to open account)
It takes approximately two weeks to open an account.

ANNUAL ADVOCACY SERVICE PROGRAM ($6100)
84 Personal Advocate hours per year
Services and Supports per the Planning Journal and Service Agreement
Ongoing maintenance of the Planning Journal
CUSTOM ADVOCACY SERVICE PROGRAM (Negotiated)
Additional/fewer hours can be provided, along with other services customized to the individual's needs.
LIFE SERVICES TRUST USE ($20,000 minimum funding)
Immediate or Future Funding
First Party (Self-Settled) and Third Party Trusts
Advocacy Services and/or Supplemental Extras
Excess Income / Medicaid Spend Down (only if in conjunction with other Life Services programs)
Annual Tax Returns for the Trust and each Trust Account
KEEPING CONNECTED ($750)
1-2 formal contacts per year (via meetings and calls), plus emergency consultations
To enable Life Services to “step-in” when needed
EXECUTOR / TRUSTEE (STAND-ALONE TRUSTS) / HEALTH CARE PROXY / POWER OF ATTORNEY (Negotiated)
Provided as need arises and with prior Board Approval.
